Sanford Health provides a global view of health within our communities through Community Health Needs Assessments. These can help align expertise and resources to address unmet health needs.

We conduct Community Health Needs Assessments to comply with the 2010 Affordable Care Act and the IRS 990 requirement for nonprofit health systems.

Sanford Medical Centers listed below completed assessments in their communities and developed strategies to address local health needs. Sanford Health values these partnerships as we all work together to improve the human condition through exceptional care, innovation and discovery. Community Health Needs Assessments were conducted during 2015-2016 and included input from a broad base of community members and leaders.
